The Mission Statement of the Certified School Nurses of Millville Public Schools is to strengthen and assist(facilitate) the educational process by improving and protecting the health status of students and staff. Goals of the Health Services Plan are: 1. Assure access to primary health care 2. Provide a system for dealing with crisis medical situations 3. Provide mandated screening and immunization monitoring 4. Provide systems for identification and resolution of students' health and educational problems 5. Provide comprehensive and appropriate health education 6. Provide a healthful and safe environment that facilitates learning 7. Provide a system of evaluation of the effectiveness of the school health services program NJFamily Care-for more information about NJFamily Care and health insurance  Mandated Physical Examinations Prior to Public School enrollment: In accordance with the New Jersey Administrative Code6A:16-2.2 and the Millville Public Schools policy, documentation of a physical examination is required for each student within 30 days upon enrolling into school. Your child's primary healthcare provider should complete this form when ordering medication to be taken in school and to identify your child's asthma triggers so everyone in school understands the steps necessary for the individual child to achieve the goal of controlled asthma. * click here for Asthma Action Plan in English Food Allergy Forms- many students now have food allergies and food intolerances. The attached forms are required to substitute foods for known intolerances and allergies. This form must be completed by your child's primary healthcare provider to allow subsitutions and omissions of foods served at school. Sports Physical Requirements: Every student athlete must submit a New Jersey Department of Education Annual Pre-Participation Physical Examination form. The form must be completed by both the parent/guardian and the student's primary healthcare provider and then returned to the School Nurse. NO STUDENT WILL BE ALLOWED TO PARTICIPATE IN SPORTS (INCLUDING PRACTICES) UNTIL MEDICALLY CLEARED BY THE SCHOOL DOCTOR. The sports physical will be valid from the date of the actual physical examination not the date the heatlhcare provider completed the paperwork. FREE SPORTS PHYSICALS ARE CONDUCTED BY THE SCHOOL PHYSICIAN AT THE BEGINNING OF EACH SPORTS SEASON! You may contact the school nurse for the specific dates of the sports physicals. The student's primary healthcare provider must first complete the sports physcial form to become eligible to participate in any intramural and/or interscholastic sport for grades 6th through 12th ( required by thre State of New Jersey). In addition to the completed physical examination, the student/parent must complete and treturn a one page sports recertification form ( Sports Update Form) before the start of the season ( fall, winter, spring sports). When to not send your child to school: Regular school attendance is necessary for optimal learning. However, the mere presence at school does not ensure effective learning. A child must be feeling well in school to maximize the learning experience. In addition, a child who is sick and comes to school may spread the illness to other students and staff. Physical Education Medical Excuses: If your child has an injury that would limit him/her from phys ed (gym) class, you must have a note from the doctor for the injury sating the limitations and the length of time needed away from the class or the next doctor's appointment. A one day excuse from gym will be granted with a parent note given to the nurse at the beginning of the school day ( or the phys ed.teacher at the beginning of the Phys. Ed period). State Mandated and Millville Public Schools School Screenings: The Nurse in your child's school will send home a letter at the beginning of the school year and to every new entrant into the school system as to what state mandated screenings will be performed on your child during the school year.
